Starting in the fall of 2022, ALL PHMS & PHHS athletes must complete their athletic forms online @ FINAL FORMS IHSAA Physical Form (A paper copy, completed by the doctor must be turned in to your coach or the athletic office prior to the 1st practice) All other forms are online at FinalForms (prairieheights-in.finalforms.com)
